Workshop for Adult Social Care to provide understanding of VOICES Multiple Needs Toolkit and how this can support in the referral process

VOICES has delivered a series of workshops to local services alongside Adult Social Care to demonstrate how to use the Multiple Needs Toolkit in practice to better communicate care and support needs when making referrals to Adult Social Care.

This opportunity is for colleagues from Adult Social Care in Stoke-on-Trent to become familiar with the Multiple Needs Toolkit in preparation of receiving referrals that include completed toolkits.
